You don’t just need to know whether a provider has capacity.
You need to know whether the participant is a suitable fit, whether someone will respond, what happens after the referral and whether communication will continue once support starts.
Safe & Smart Care aims to make that process clear from the beginning.
Safe & Smart Care has consistently passed its NDIS audits with no
non-conformities. We're proud of this result because it reflects the
strength of our systems, the quality of our service delivery, and our
commitment to safe, accountable and participant-centred support.
Accepting new participant referrals across all registered service areas.

WHAT YOU CAN EXPECT
A referral shouldn’t disappear into a system.
01
A clear suitability decision
We review the participant’s support requirements, location, timing and our current capacity before confirming fit.
02
Thoughtful matching
Participant needs and preferences are considered when establishing the support-worker relationship.
03
Clear communication
You should know the status of the referral, what happens next and who to contact.
04
Follow-through
Commencement does not end our responsibility.
Where appropriate and consent allows, communication can continue after support begins.
REFERRAL PROCESS
From referral to support — without the guesswork.
01
Send the referral
Complete our referral form or contact the team directly.
02
We assess fit and capacity
We review the required service, location, timing and current capacity.
03
We speak with the participant or representative
We learn more about the person, their needs, preferences and expectations.
04
We confirm the support arrangement
If Safe & Smart Care is the right fit, we clarify the service requirements and work through an appropriate support-worker match.
05
Support begins
The participant knows what has been arranged and what to expect.
06
We stay involved
Communication continues and issues can be raised as the support relationship develops.
Our referral pathway is supported by documented matching, communication and escalation procedures. We assess the participant’s needs, preferences, goals, location, cultural background and required worker skills and experience, and record and follow up communication with participants, families, representatives and referral partners.
